This is the intake paperwork for the Batters Intervention and Prevention Program (BIPP). Once these are received, a facilitator will then set up your intake meeting on a Zoom call. If you leave the page before you click submit, you will lose your answers. Please complete a step before leaving by clicking SUBMIT. You can submit a step, come back and continue on with the remaining steps. Please note, all 10 steps must be filled out to initiate an intake.
You also must pay your $50 intake fee prior to the Zoom call with the facilitator. This can be done several ways: You can pay here in your member account through PayPal, you can pay through Venmo (@Crisis-Center) on your phone app, or you can pay cash at the Crisis Center (Mon-Fri 8am-5pm).
